Responsibility：
- Provide community occupational therapy services to SDHC clients in Core Centre, Satellite Centres, and outreaching services to the community including health assessment and screening, health coaching, planning and implementation of chronic diseases management and community rehabilitation programmes
- Collaborate with SDHC multi-disciplinary team to develop and conduct health promotion and patient education programmes and related training
- Develop clinical guidelines and workflow of occupational therapy services
- Work with multi-disciplinary professional teams to deliver primary health care services
- Collaborate and liaise with different stakeholders including government officials, network medical practitioners, professional groups, district stakeholders and related parties
- Provide occupational therapy services in Innovative Projects
Requirement：
- Registered as Occupational Therapist in Part II of the Register of Occupational Therapist under the Supplementary Medical Professions Ordinance and have a valid Practicing Certificate issued by the Occupational Therapists Board, Hong Kong
- Degree in Occupational Therapist or equivalent
- Minimum 2 years of aggregate experiences as Registered Occupational Therapist in patient care related occupational therapy practice, preferably with experience in community healthcare service in public services/ NGOs
- Recognized post registration training in Primary Health Care/ Community Care / Diabetes Mellitus care/ Cardiology/ Stroke Rehabilitation/ Cardiac Rehabilitation/ Gerontology are advantages
- Good communication, organizational and interpersonal skills with strong team work
- Excellent command in spoken and written English and Chinese
- Shift duty including 1-2 days night shift until 9:15pm per week is required
